If you've formed an AI governance committee, you made the right call. But structure alone doesn't govern anything. A committee that meets and discusses but never decides is just a talking shop. The real work is answering the same question over and over: Should we approve this AI decision or reject it, and why.

A framework rests on four main pillars to manage the lifecycle of an AI model.
1. Transparency and explainability: AI shouldn't be a black box. A framework ensures that developers can explain why a model made a specific decision. This matters in high-stakes fields like healthcare or lending, where decisions directly affect people.
2. Fairness and bias mitigation: Since AI learns from human data, it inherits human prejudices. Governance frameworks require regular audits to check for bias against specific demographics. This keeps the math objective.
3. Accountability and compliance: This establishes who's responsible when things go wrong. It means mapping the AI to global regulations like the EU AI Act, NIST AI Risk Management Framework, GDPR.
4. Safety and security: This protects the model from adversarial attacks (hackers trying to trick the AI) and ensures the system doesn't cause physical or digital harm if it fails.

Is it transparent and accurate?
Can you explain how the model works? Can you prove it's accurate? Transparency starts with data. What did you train on? What metrics prove accuracy? Accuracy testing means using holdout data (data the model has never seen) to validate performance.
If a model performs great on training data but fails on new data, it's overfit and dangerous.
Your team proposes deploying a churn prediction model to identify customers likely to leave. During the review, the committee asks:

Is it fair?
Fairness means the model treats demographic groups equally. Demographic parity measures whether the model makes the same decision rate for all groups. Equalized odds measures whether all groups have equal true and false positive rates.
These aren't perfect metrics. But they're measurable, and measurable beats nothing.
Does the model predict equally well across geographies? You find 78% parity for European customers versus 92% target. Committee requires expansion of training data or monthly monitoring. Without this check, the model launches broken for an entire segment you didn't even know about.
Will it stay accurate and safe?
Set re-evaluation triggers on accuracy drift or fairness drift. Data drift means real-world data differs from training data. Check monthly for high-impact models. Quarterly for lower-risk ones.

Most companies have no governance on vendor AI tools. You adopt a third-party tool and feed it proprietary data without asking basic questions.
This is where governance matters most.
The committee evaluates three areas mapped to the four pillars.
Transparency and accountability: Does the vendor train on your data?
This should be one of the first questions an AI governance committee asks. Some vendors use customer data to improve their models, but the terms are not always obvious upfront.
Ask directly:
Get the answers in writing.
If the vendor cannot clearly answer yes or no, or avoids the question entirely, treat it as a governance risk. Your customer conversations, sales activity, and business metrics should not automatically become training material for a third-party AI system.
Example:
Sales wants to deploy an AI forecasting tool that claims 92% accuracy. Committee asks:
Committee approves with conditions to verify data residency commitments in the contract and require quarterly audits.
Safety and fairness: Does it meet your security and bias standards?
Ask about vendor security: Data encryption, access controls, audit logs. Ask about bias testing: Do they test for bias in their models? How? What's their monitoring process?
If they can't answer, that's a signal they haven't thought about fairness.
Accountability: What happens if the vendor changes terms or fails?
Ask about SLAs (service level agreements). What uptime do they guarantee? What's your recourse if they breach it? What's their data retention policy if you leave Can you export your data?

Write standards before you approve individual decisions. When standards exist, decisions are faster because everyone knows the bar.
Sometimes a decision doesn't fit the standard. That's where exceptions come in. An exception is a documented request that goes to the committee with business justification. It requires written approval, not a vague handwave.
Your churn prediction model hits 88% fairness (demographic parity), below the 92% threshold. Customer Success needs to deploy it next week.
This is an exception request.
Committee asks: Why can't you wait for a retrained model? Customer Success: Churn rates increased sharply this quarter, and the team needs predictive signals now to identify at-risk accounts and intervene early. Committee documents: "Approved for 4-week deployment with weekly fairness monitoring. Plan to retrain with more data and re-evaluate."
This maintains accountability while allowing business flexibility.
Document every exception in writing: What standard is exceeded, why, for how long, and what mitigation exists. Without documentation, exceptions become the norm and governance disappears.

Document at each step: What was requested, who reviewed it, what was the decision and why, was there dissent, what triggers re-evaluation.
Use a simple decision log. Date, decision, owner, approval or rejection, reason, monitoring. A spreadsheet works.
The moment a decision is made, notify relevant teams. Governance fails when decisions are made in a room and ignored by the people implementing them. If a decision is unworkable, teams should push back immediately so the committee can adjust it.
Governance frameworks don't slow you down if they're designed right. They speed you up by removing ambiguity.

Copy these templates. Fill in your standards. Start with one decision type. Build from there.
You don't need to calculate fairness metrics yourself. Ask your model vendor or development team to run the analysis and report results. You set the threshold (for example, "demographic parity must be 90% or higher") and they validate against it. If you're building models internally, hire a data scientist or consultant to help define metrics appropriate to your use case.
Start with three checks: Does the vendor train on your data? (Critical.) Is there a data processing agreement in place? (Required for GDPR and CCPA compliance.) What's the uptime SLA? (Matters for business continuity.) Once you have governance muscle, add fairness testing and security audit questions. Don't boil the ocean on day one.
Document every exception formally. Include: which standard is being exceeded, business justification, approval date, expiration date, and mitigation. Review exceptions monthly. If the same exception request comes up three times, your standard is probably wrong. Revise it. But document the revision. Don't let exceptions erode standards silently.
Product or the AI team lead. This person needs enough seniority to commit their team's resources and own the outcome if the model fails. They listen to input from engineers (feasibility), risk and compliance (thresholds), and legal (liability), but they decide. If dissent is severe or risk threshold is exceeded, escalate to the CEO. The owner decides, owns the outcome, and moves on. Engineers who deploy or use the model should provide reality checks on what's actually executable. At minimum, monthly for high-impact models (revenue-facing, customer-facing). Quarterly for lower-impact ones. Also re-evaluate if accuracy drops 5% or more or fairness metrics drift more than 10%. Set these triggers in advance so teams know when to flag issues.
